FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is searching for a Social Services Manager to oversee and coordinate all hospice social worker and chaplain daily activities. The ideal candidate will be organized and focus driven to ensure social work and spiritual care standards of care are implemented and assist in carrying out hospice social worker duties as needed.

We believe in a professional team approach and candidates must demonstrate great communication and social interaction skills, as well as, be a compassionate individual to work with patients, families, and other professionals to promote the philosophy of hospice and palliative care.

FAIRHOPE is seeking experienced social workers with healthcare background. The ideal candidate for Social Services Manager works to maintain knowledge of and understands the ever-changing trends in healthcare and hospice/palliative care.

Responsibilities

  • Provide management and supervision of Social Services team, which includes the Hospice Social Workers and Spiritual Care Coordinators.
  • Coordinate initial consultations/admissions by Hospice Social Workers in collaboration with Admissions Team to ensure timely completion. Make initial consultation visits to explain hospice and palliative care programs and philosophy as needed.
  • Participate in the development of the care plan. Assess the psychosocial status of patients/families/caregivers related to the patient’s terminal illness and environment. Assess the patient’s identified residence and provide assistance when it is not safe and another plan in required.
  • Carry out social evaluations and plan intervention based on evaluation findings. Counsel patient/family/caregivers as needed in relationship to stress and other identified coping difficulties.
  • Assess volunteer needs of patient/family and complete volunteer referral as appropriate.
  • Maintain clinical records on all patients. Document accurate and relevant clinical notes regarding phone calls, visits, and care/services provided.
  • Coordinate discharge planning and participate when needed. Assist patient/family/caregiver with securing advance directives and funeral arrangements, as needed.
  • Develop collaborative relationships with organization personnel to support patient care. Maintain and build contacts with public and private agencies as resources for patient and as needed for personnel. Provide information and serve as liaison for patients, families and/or caregivers on community agencies.
  • Attend and actively participate in interdisciplinary group (IDG) meetings.
  • Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
  • Participate in the selection, orientation, ongoing staff training, job performance evaluation, and discipline of Home Care team members.
  • Serve as Administrator On-Call services as scheduled.
  • Assist in the development of organizational goals. Assist in development, recommendation, and administration of organization policies and procedures.

Qualifications

  • Graduate of a Social Work program accredited by the Council on Social Work Education. MSW preferred.
  • Currently Licensed Social Worker/Licensed Independent Social Worker licensed to practice in the State of Ohio.
  • Management experience. Minimum experience of two years in a health care setting with preference of hospice and palliative care.
  • Understand the philosophy of Palliative Care and Hospice concept with emphasis on understanding the needs of the terminally ill.
  • Ability manage time efficiently and analyze and implement proper action is essential. Demonstrate great communication and social interaction skills, as well as a compassionate individual.
  • Computer skills required.
  • Must have and maintain a valid driver’s license, good driving record, and auto accident insurance at a level acceptable to FAIRHOPE and FAIRHOPE’s insurer.
  • Physical Setting: In-office environment and various healthcare settings. Some driving may be required.
  • Work Location: Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties. Main office location is located in Lancaster, Ohio.
